1.   The charges follow a hammer attack on an off-duty policeman in Cheltenham in January.

2.   Scott Douglas apparently jumped off the Tappan Zee Bridge after the hammer attack on his wife.

3.   A psychiatric outpatient said she wanted to scare her caseworker, not kill her, when she planned and followed through with a hammer attack during a home visit.

4.   She said the inspection would have been done anyway, but was moved up by about six months because of public concern following the hammer attack.
