Furthermore, health authorities could no longer discharge patients without an individual care plan being approved and agreed with the local authority. 6. The addition of the care plan will enable service agreements to be tailored to meet the needs of the individual user. 7. The products and the care plans they produce are more in-depth. 8. This requires a measure of continuity in ward staffing, to make sure that nurses and support workers know individual patients and understand the care plan. 9. Management was not built around care plans and reviews... and there was little involvement at senior level. |
